Guide to Living Well

This guide is for you! Hello, and welcome to CarePartners, where you are the most important part of your health care team. Our health professionals have clinical training; however, you are the expert when it comes to your body, your health, and your needs.

We will provide you with the information you need to understand your condition and to make choices about your lifestyle and treatment. In return, we need you to share with us when your condition changes, when your medication or medical care changes, and when you have questions or concerns. Together, we will develop a plan for care and adjust things as they evolve. Our relationship is a partnership.

If you have questions, concerns, or feedback, please contact the office you normally deal with (closest to the patient’s home address).
