CarePartners has on staff over 600 compassionate and highly competent Registered Nurses and Registered Practical Nurses. Our nurses deliver patient and family centred care across the province in many community settings including clinics, homes, schools, workplaces and long term care facilities. CarePartners nurses work closely with the entire health care team - this team includes the patient, their family, home care service providers, the Ontario Health atHome Coordinator, the physician and other health professionals.
CarePartners nurses provide care in accordance with best practices; this means that in collaboration with the patient and the health care team.
Our nurses provide care that research shows achieves the best results, results such as the healing of a wound or the successful management of a chronic disease.
CarePartners nurses also place high importance on teaching patients and families how to self-manage their health conditions and become independent in their care. All CarePartners nurses have experience in coaching patients and families to become very proficient in performing nursing skills including learning complex tasks such as administering IV medications or giving injections. There is a lot of satisfaction on both the part of patients and nurses when patients become the experts in their care and the nurses start to learn from the patients!
